Agreement Form

To ensure safety and enjoyment of the activities, we ask everyone to read and sign the hard copy of the waiver form at Teru-Neko-Ya.

1. I understand that Teru-Neko-Ya (cat-cafe) has available certain cats to be played with for certain fees and rules bound by its terms.
   
2. The activities may not be supervised by the staff at all times, but will remain in the premises to monitor and offer guidance.
   
3. I am in sound physical and emotional condition to play with the animals and others in a safe, appropriate manner.
   
4. I will follow all rules and undertake all activities in a responsible manner.
   
5. I am aware of the inherent dangers involved in playing with the cats, health risks, injures, damages to self and others. Participants must take sole responsibility and will not be covered under the insurance of Teru-Neko-Ya, and the customers do not have any right to file claims against the cat-cafe.
   
6. If I am unwilling or unable to follow any restrictions, the cat-cafe can terminate my activities without any refund.
   
7. I agree to pay all fees and costs caused by an intentional wrongful act arising as a result of my presence at the cat-cafe.

*I am the supervising parent/legal guardian of the minor(s) listed below. I accept on behalf of the minor(s) all the rules and will take full responsibility of their actions.
